Call or stop by for an appointment.  New patients can be seen within 24 hours.  

Please:

• Bring your current insurance cards
• Bring your prescription for Physical Therapy
• Arrive 15 minutes early for your first appointment to complete paperwork
• Wear appropriate clothing to allow access to painful areas

Initial Evaluations require 1 to 1 ½ hours, regular visits 45 min to 1 hour.

Handicap parking is available. We are wheelchair and ADA compliant.

Copayments are expected prior to every visit. We accept Visa, Discover, Mastercard, personal checks, and cash.
